Birthday Party Reservation Form

There is a $25 nonrefundable deposit required to reserve you party. And a $10 per person (kids & adults) admission fee. (Birthday person is free.)
This includes:

• 2 hours at a reserved location including one picnic table with straw bales in a semi-circle for seating.
• Our Birthday Coordinator to assist you with onsite details
• One Fast Pass for a group hayride  (so your group will not have to wait in line)
• Birthday song/sing-a-long at the music tent for the birthday person
• A wristband for each person (good for entire day) which includes:
  • UNLIMITED Jumping pillow
  • UNLIMITED Hayrides
  • UNLIMITED Corn Maze
  • UNLIMITED Straw maze
  • UNLIMITED Petting Zoo
  • UNLIMITED Tire Mountain
  • UNLIMITED Play Areas
• Wheel chair accessible (Please call ahead if possible)

BIRTHDAY PARTY MEAL PACKAGES:

• Hot Dog Meal $5 - includes hot dog, chips, and a drink
• Brat Meal $6 - includes brat, chips, and a drink
• Pizza Party $42 - includes one 18"x26" (16 pieces) and 10 bottles of water. (sausage, pepperoni, or cheese. no 1/2's)

We welcome you to stay at the Pumpkin Farm and explore the farm for as long as you wish.
After your two hour time frame is up; please thoroughly clean up the area (especially the ground). Thank you!
